Survey Rating

Hello Support
I have one hospital who uses the HSN and now want to take survey from patients and clients. Their request to be able to ask question where the type will be and option Scaling.
HNS has Option and Scale but that is sometime limits their mode of questioning the customers.

Below is an Example of their Questions

Nursing care (where applicable)
How would you rate the following?
Professionalism Excellent Good Fair Poor
Prompt response to requests Excellent Good Fair Poor
Understanding of your needs Excellent Good Fair Poor
Overall impression Excellent Good Fair Poor

Laboratory Service (where applicable)
How would you rate the following?
Promptness of response Excellent Good Fair Poor
Professionalism Excellent Good Fair Poor
Understanding of your needs Excellent Good Fair Poor
Overall impression Excellent Good Fair Poor


The above is an example of what they want where on question can have option to scale answers at a goal.
Before Introducing them to HSN they were using Surveymonkey from surveymonkey.com where they can create questions in that format.

I am not able to do that with HSN at the moment. Can you please help me in any way to pleased my customer?

Hi Courage Smith Asilevi,
you can accomplish your request by using:
- Rating scales
- Option

Rating scales: you can only use stars so you have no mode to insert text.
Option: you can manually insert text in every single option.

You can make different surveys divided per question, and enable Surveys in "Enable, all those provided" mode under your domain, or make a single long question.
